A “systematic breakdown” of safety management processes by the operator of the Luas was detected during an audit by the railway safety watchdog last year.

The Commission for Railway Regulation (CRR) concluded that, while safety standards and procedures existed, they were not fully complied with and “not being managed appropriately”.

The audit found that Transdev, the company that operates the Luas system, had kept no record of any safety incidents in 2018 or 2019 in its risk register. The CRR had been notified of 38 incidents last year, including two fatalities, and 25 incidents in 2018.

The auditors said the failure to record incidents on the risk register meant Transdev was not in a position to assess any trends in relation to potential safety issues.
